What is hair loss?

Hair loss, also known as alopecia, refers to the partial or complete loss of hair from areas of the body where it normally grows. In men, the most common form is androgenetic alopecia, also known as male pattern baldness. This is a hereditary condition that causes hair to gradually thin, typically starting at the temples and crown, and progressing over months or years. Hair loss can also result from medical conditions, medications, stress or nutritional deficiencies. While some hair loss is temporary and reversible, male pattern baldness is progressive, meaning it will continue without treatment.

What causes hair loss in men?

The most common cause of hair loss in men is male pattern baldness, which is driven by a combination of genetics and a hormone called dihydrotestosterone (DHT). DHT is produced from testosterone by an enzyme called 5-alpha reductase. In genetically predisposed men, DHT causes hair follicles on the scalp to gradually shrink in a process known as follicular miniaturisation. Over time, the affected follicles produce thinner, shorter and lighter hairs until they eventually stop producing visible hair altogether. Other causes of hair loss include thyroid disorders, iron deficiency, autoimmune conditions such as alopecia areata, significant physical or emotional stress, and certain medications.

What are the signs of hair loss?

The early signs of male pattern baldness can be subtle and easy to miss. Common indicators include a receding hairline, particularly at the temples; thinning hair on the crown or top of the head; more hair than usual on your pillow, in the shower or on your comb; and a widening of the parting. Hair may also feel finer or less dense than it used to. Male pattern baldness typically follows a recognisable pattern,  it begins with thinning at the hairline and crown and gradually progresses. The earlier you seek treatment, the more effective it is likely to be, because it is easier to maintain existing hair than to regrow hair that has already been lost.

What are the different types of hair loss?

There are several types of hair loss, each with different causes and characteristics. Androgenetic alopecia (male pattern baldness) is by far the most common in men and is caused by genetics and hormones. Alopecia areata is an autoimmune condition that causes patchy hair loss, sometimes affecting the entire scalp or body. Telogen effluvium is a temporary form of hair loss triggered by stress, illness, surgery or significant hormonal changes. The hair typically regrows once the underlying cause resolves. Traction alopecia results from repeated pulling or tension on the hair. Scarring alopecia is caused by inflammation that destroys hair follicles permanently. What hair loss treatments are available?

The two MHRA-licensed treatments for male pattern baldness in the UK are finasteride (a prescription-only oral tablet) and minoxidil (a topical solution or foam available over the counter or on prescription). Finasteride works by blocking the conversion of testosterone into DHT, the hormone responsible for follicle miniaturisation. It is taken as a daily 1mg tablet and has been shown in clinical trials to slow hair loss and promote regrowth in the majority of men. Minoxidil is applied directly to the scalp and works by improving blood flow to the hair follicles, stimulating them into the active growth phase. How long does hair loss treatment take to work?

 Hair loss treatments require patience and consistency. Finasteride typically takes three to six months of daily use before visible results become apparent, with maximum benefit usually seen at 12 months. Some men notice a reduction in hair shedding within the first few months. Minoxidil follows a similar timeline, with most users seeing improvement between three and six months. It is important to continue treatment consistently. If you stop taking finasteride, DHT levels return to normal and hair loss will resume, typically within six to 12 months. Treatment works best when started early, before significant hair has been lost.

Can medication cause hair loss?

 Yes. Several prescription and over-the-counter medications can contribute to hair loss as a side effect. These include certain blood thinners (anticoagulants), beta-blockers used for high blood pressure, some antidepressants, retinoids used for acne, chemotherapy drugs, and hormonal medications, including anabolic steroids. Hair loss caused by medication is usually a form of telogen effluvium, meaning it is temporary and hair typically regrows once the medication is stopped or changed. If you suspect that a medication is causing hair loss, do not stop taking it without consulting a doctor; they can advise on alternatives or adjustments.

Are there side effects of hair loss treatment?

Finasteride is well tolerated by most men, but a small number may experience side effects, including reduced libido, erectile difficulties or breast tenderness. These effects are uncommon and typically resolve after stopping the medication. The MHRA has also issued advice that finasteride may be associated with changes in mood, including low mood, depression and, in rare cases, suicidal thoughts. If you notice any changes to your mental health while taking finasteride, stop the medication and speak to your prescriber straight away. Minoxidil can occasionally cause scalp irritation, dryness or unwanted facial hair growth if it comes into contact with the face. Some men experience a temporary increase in hair shedding during the first few weeks of treatment; this is a normal response as weaker hairs are shed to make way for stronger new growth.
